Wife needs better mattress and ramp for Chair

My wife, Samantha is a mother of 2 and a former Nurse, who has Fibromyalgia, PTSD, Bipolar disorder, kidney failure, and Rheumatoid   Arthritis.  Due to these conditions, she has had to medically retire from nursing at only 33 years old.  

Many days, she finds it hard to move from the pain.  On the days we go out, (like to do what food shopping we can afford), she can only walk a few isles and then she starts sweating, and has to find a place to sit down.  We used to be a very active couple, would go walk around malls and zoos, now we can't do that anymore.  This makes her depression worse, because she feels she is trapped in the home.  

We were able to get a used powerchair for her for her bad days, but no ramp to get it into the back of the car (about a 4 ft difference), so we can't use it.  The chair weighs about 180 lbs, so I can not lift it.  On top of that, we have had our current mattress for over 10 years and it is so bad, it makes her pain worse.  

Due to having a low paying job, I can barely pay our bills, much less a mattress or ramp.   ON top of that, our car needs some repairs to make it good for us to use for this.  

If I can get her out more, her depression and feeling like she is trapped in the house would lighten.

I would like to be able to get her an adjustable or memory foam mattress so she can sleep in a more comfortable position for her pain, and a ramp so we can go out and go places we cannot now.

I love my wife she is a very special person to me and I am asking for help so I can give her what she needs.  It is embarrassing to have to beg for help, but I would do anything for her to feel better.
