proc Left ankle fracture needed internal fixation 1 of 1 Ineel Hospital Skdan. Kraetobme 3 Box 7442 28 February 2019 To whom it may concern Re: Mrs. Nagat Abdalwahab Abdalla, 52 years Problems: Type C Left ankle fracture needed internal fixation Secondary wound infection Compound Right distal Tibia and fibula fracture 2006 Hypertension Diabetes type 2 Asthma Medication: Metformin Amlodipine Losartan Aspirin Symbiont turbohaler BD Salbutamol inhaler PRN Mrs. Abdala came to see me today for review and repeat x-rays. I've seen Mrs. Abdalla first in August 2017, after she sustained an injury to her left ankle and found to have type C left ankle fracture for which she needed an operation, I took her to theater on 20/8/2017, for internal fixation, ankle joint was stabiized with plates and screws on both medial and lateral sides of the foot. Post -operative x ray was satisfactory. Unfortunately she developed wound infection few days after surgery and the healing was slow given her medical history of diabetes, she required prolonged course of IV Antibiotics with some improvement Sadly this has affected her mobility and resulted in delay in mobilization after the surgery. Of note Mrs. Abdalla has a history of compound fracture on the right distal tibia and fibula in 2006, which was angulated and needed reduction and internal fixation. And I think this has contributed to delay mobilization as well. Today she told me that her joints have become more stiff the last 12 months or so and she can hardly weight bear on her legs after I examined her I found both joints to be stiff with restricted range of movement on flexion and dorsi -flexion and inversion . The left foot was swollen with some tenderness on lateral side. She is currently bound to wheel chair and she requires continuous assistance to be able to perform her daily activity of living, she has extreme difficulty getting out of the bed and requires significant amount of help to go to the toilet and bathroom and to perform other daily activities Her feet often get swollen and painful and she takes regular analgesia to alleviate the pain and discomfort. She also suffers from diabetes, hypertension and Asthma and she takes the above mentioned medication and requires regular monitoring of her blood pressure and sugar control and she always needs somebody to help with taking her regular medications.
