Help with a long term fight with Crohn’s and other medical issues

Hi, my name is Jacob. I'm 30 years old and I've been battling with Crohn's disease for almost 2 decades now. I've been hospitalized, have had surgeries, tests, procedures, endless amounts of medications and treatments. Thanks to my Crohn's I also have blood clots, insomnia, psoriasis and arthritis. For the most part I've been doing this on my own and after all of the years of fighting I unfortunately need to ask for help. My dad died during back surgery when I was 12 right around the same time that I first started having issues with my stomach. I've always tried my best to manage and maintain my health and sanity along with trying to stay positive. I had a rocky relationship with my mom, she tried her best but we didn't always see eye to eye. She unfortunately passed away almost 2 years ago due to colon cancer and a brain tumor. She didn't have much nor did she have life insurance or health insurance so I was left with the bill for her service on top of all of my medical bills. Like I said, I have always tried my best to manage everything but due to several circumstances l'm still drowning in medical debt, to make things worse I have maxed out my credit cards and taken out a loan just to try to stay afloat. I've always been a hard worker, for the majority of my life I've worked 2 jobs putting in anywhere from 40-90 hours a week. I have 2 decent jobs but between my medical, rent, car and living expenses I just can't catch a break. Thanks for your time and consideration.
