The theory is that disc replacement, rather than fusing the disc space, maintains more normal movement in the lumbar spine, reducing the possibility that adjacent levels of the spine will deteriorate from increased stress. This procedure is still a new procedure in the US, so long-term efficacy and potential risks and complications are still relatively unknown.
The main concern with artificial disc replacement is that anything that moves can wear out over time. Many patients who undergo DDD surgery are younger than patients who have artificial hip and knee joints placed, and although there is less wear on a vertebral joint than on a major joint such as the hip or knee, with the time, there may be a considerable risk that these artificial discs will need to be replaced in the future.
Artificial discs are placed through an anterior approach to the spine under the large blood vessels (aorta and vena cava). After the procedure, these vessels heal down the spine, making revisions problematic.
