What Happened :

I'm writing to you on behalf of Glenn Paul Bongulto (Patient’s Name) at SAN JUAN DE DIOS EDUCATIONAL FOUNDATION, INC- HOSPITAL, admitted yesterday, June 15,2019 around 6pm, where he received laboratory tests and on going (CBC/ Creatinine /CITI SCANwith chronic liver disease) specified procedure or treatment, etc,. He have no insurance covered at this time because started his new job last March 21, 2019 only .

He is 25 years old, a brother of 4 siblings and the sole breadwinner of the family. Putting his siblings through schooling or helping them became substantial. Also, he have no other income beside his job. Glenn’s father has no permanent work and his mother is just a plain housewife. The elder sister who’s role is a secondary provider of their family, but the salary get is not enough to pay the other finances. He have unfortunately run into health issues that causes hospitalizations and medications. He have lost his job and no income coming in, and making payments is becoming difficult for them.
