Transportation to Medical Appointments!

I am on disability, a fixed income and after paying my Medicare Advantage Plan I make $650 a month. Still trying to get into low income housing. Need to get back and forth to city to get medical care that they don't provide in my small town. I can't even find a doctor in this town that will take me on as a patient because of my medical problems, I have herniated, torn discs, and nerve damage in my back, I have an enlarged heart and an enlarged prostate. I can't even get the basic health care in this town. The only reason I am living here is the cost of living is much lower and I am on the list for fixed income housing. It has already dropped below freezing temperatures and I help a single mother friend of mine out by taking her kids to school everyday and it is getting to cold for me to do that because my back locks up when it gets cold. Any help would be appreciated please anything.
