Dear family, friends and everyone,
At the young age of 34, Florian, my husband of 12 years, was diagnosed with terminal rectal cancer (stage 4) with liver metastases on 21 May 2019, just one day before our youngest son’s second birthday. Florian is a wonderful husband, father, son and friend. He is kind, caring and always ready to help others. The news came as a shock and a great blow to our family. I still do not know how to break this news to our three young children, Caleb, Chloe and Cyrus, who are only 6, 4 and 2 years old.
Florian has always been healthy for as long as I have known him. When he found blood in his stool in last December, no one could have suspected it was anything serious. When bleeding finally became constant in April he underwent a colonoscopy after consulting a gastroenterologist. The lab result showed that he has a tumor in his rectum which is around 4cm large. Recommended by the doctor he subsequently went for CT and MRI scans, which found multiple shadows in his right lope liver. A PET scan confirmed 6 metastases in the liver.
We have consulted a couple specialists for rectum and liver surgeries and two oncologists. The estimated medical fees are overwhelming to us. Florian will need three surgeries in total, which include rectum and liver resections and ileostomy reversal, followed by six months of chemotherapy with targeted therapy. PET scans will also be needed regularly during chemotherapy to check how his body responds to the drugs. Further treatments may be required after the chemotherapy.
THE COSTS
We were told that the three surgeries will cost approximately HKD600,000 in total. The chemotherapy with targeted therapy will be around HKD1,560,000 for six months. These are only for the initial treatments and do not include all on-going costs including doctor’s consultation fees, scans and tests. The breakdown of the medical costs are as below:
Rectum and liver resections = HKD520,000
Ileostomy reversal = HKD80,000
Chemotherapy for six months = HKD960,000
(HKD80,000 per dose x 12 doses)
Targeted therapy for six months = HKD600,000
(HKD50,000 per dose x 12 doses)
*The costs for chemotherapy and targeted therapy vary depending on how his body responds. But these are the treatments suggested by his oncologist for now.
His insurance is only covering a fraction of the cost - 10 to 15% of the estimated costs depending on how the various fees are classified - and the rest will be self-finances. My husband was referred to Queen Mary public hospital, he is registered there and on the wait list. The public hospital in Hong Kong has a long waiting list. Even given the urgency of this case, my husband was given a waiting time of 4 to 6 weeks for the surgeries, and we have not been given an appointment with an oncologist from the public hospital so far and are told to wait for his call, adding a further 4 to 6 weeks. Nevertheless, we are told that the government might or might not cover the costs for chemotherapy and targeted therapy depending on the drugs he will need.
URGENCY
None of the doctors we met was willing to tell us of his life expectancy, not even an estimate. According to articles I read on the website of the American Cancer Society, the average life expectancy for terminal rectal cancer with liver metastases without treatments is around six months, which makes time a critical factor. We do not want Florian to lose his life because we are unable to afford private surgeries and therapy treatments. The longer he waits the higher chance for his cancer to spread to other organs which might make it no longer operable or curable. He cannot afford to lose another day without having medical treatment. He has already lost 8kgs since last December. He has lost his appetite, his constant diarreha and rectal bleeding are the most concerns for us.
NO OTHER OPTION
We have also considered the option of returning to Switzerland, of which he is a national, and let him get treated there. But we don’t know for sure that the government will cover all his chemotherapy and target therapy treatments. We also do not know how fast they will be able to operate on him and give him chemotherapy subsequently. We are told by the Swiss authority that returning for the sole purpose for medical treatments is not allowed, he must first relocate back and get registered, all the administration work will be time consuming as well. His family is already working on finding out the answers. Given the urgency, he cannot lose another day or month to wait for the treatments he needs. On top of that, with no work and income in Switzerland, we will be facing more problems. Also relocation means a huge bet on his life, his life insurance provided by his company will be cut off and this is what he concerned most for us. He is very worried how we are going to live by if he passes away as he is the sole bread winner in the family.
His company now pays him 120 days of sick leave with full salary and then deducted salary (2/3 and 1/3) for another 120 days. After that we will have no more income to live on. We do not know yet if he will be able to work during chemotherapy and if not he is unlikely to be able to return to work within six months. Besides having to cover all his medical expenses, we also need money to cover our general expenses including rent, food, utilities and transportation. There are many unknowns to us at this point, but we are doing what we can now to get him treated first.

Liebe Familie und Verwandte, Freunde, Bekannte und Unbekannte
Im Alter von nur 34 Jahren wurde bei Florian, meinem Ehemann, am 21. Mai 2019, nur einen Tag vor dem zweiten Geburtstag unseres jüngstes Kindes, ein terminaler Rektumkrebs mit Lebermetastasen diagnostiziert. Florian ist ein wunderbarer Ehemann, Vater, Sohn und Freund. Er ist nett, fürsorglich und immer bereit, anderen zu helfen. Die Nachricht war ein Schock und ein schwerer Schlag für unsere Familie und Angehörigen. Ich weiss immer noch nicht, wie ich unseren drei kleinen Kindern Caleb, Chloe and Cyrus, die erst 6, 4 und 2 Jahre alt sind, diese Nachricht überbringen soll.
Florian war gesund und aktiv seit ich ihn kenne. Als er im vergangen Dezember Blut in seinem Stuhl fand, konnte niemand ahnen, dass es sich um etwas Ernstes handelte. Als sich die Blutungen im April häuften, unterzog er sich nach Rücksprache mit einem Gastroenterologen einer Darmspiegelung. Das Laborergebnis zeigte, dass er einen 4cm grossen Tumor im Rektum (Darm) hat. Auf Empfehlung des Arztes, ging er anschliessend zu CT- und MRT-Untersuchungen, bei denen mehrere Schatten in seinem rechten Leberlappen gefunden wurden. Ein PET-Scan bestätigte unsere Befürchtungen: insgesamt 6 Metastasen (Ableger) wurden in der Leber festgestellt.
Wir haben ein paar Spezialisten für Rektum- und Leberoperationen sowie zwei Onkologen konsultiert. Die geschätzten medizinischen Kosten sind überwältigend für unsere junge Familie. Florian wird sich insgesamt drei Operationen unterziehen müssen, darunter Rektum- und Leberresektionen sowie Ileostomieumkehr, gefolgt von einer sechsmonatigen Chemotherapie mit gezielter Therapie. Zudem werden während der Chemotherapie regelmässig PET-Scans benötigt, um zu überprüfen, wie sein Körper auf die Medikamente reagiert. Weitere Behandlungen nach der Chemotherapie können zu diesem Zeitpunkt nicht ausgeschlossen werden.
KOSTEN
Uns wurde gesagt, dass die drei Operationen insgesamt ca. CHF 75,000 kosten werden. Die sechs-monatige Chemotherapie mit gezielter Therapie wird auf rund CHF 200,000 geschätzt. Dies beinhaltet jedoch nur die Erstbehandlung und deckt nicht alle laufenden Kosten ab, einschliesslich der Gebühren für die ärztliche Beratung, Scans und Tests. *1CHF ≈ 7.9HKD
Die Aufstellung der privaten medizinischen Kosten ist wie folgt:
Rektum- und Leberresektionen ≈ CHF 65,000
9Ileostomieumkehr ≈ CHF 10,000
6-monatige Chemotherapie ≈ CHF 120,000 (~CHF 10,000 pro Dosis x 12 Dosen)
Gezielte Therapie für sechs Monate ≈ CHF 80’000 (~CHF 6,666 pro Dosis x 12 Dosen)
* Die Kosten für Chemotherapie und gezielte Therapie variieren je nachdem, wie sein Körper reagiert. Aber dies sind die Behandlungen, die sein Onkologe derzeit vorschlägt.
Florian’s Versicherung (über seinen Arbeitsgeber) deckt nur einen Bruchteil der geschätzten Kosten ab, voraussichtlich 10 bis 15%, je nachdem, wie die verschiedenen Gebühren klassifiziert werden. Die restlichen Kosten müssen wir aus Eigenmitteln finanzieren.
Das öffentliche Krankenhaus in Hongkong hat eine lange Warteliste. Trotz der Dringlichkeit beträgt die Wartezeit für die Operationen mindestens 4 bis 6 Wochen. Zum Zeitpunkt dieses Schreibens warten wir noch immer auf einen Termin bei einem Onkologen des öffentlichen Krankenhauses. Dies kann weitere Wochen dauern. Zeit, die Florian nicht hat. Zudem besteht keine Garantie, dass die öffentliche Hand die Kosten für Chemotherapie und gezielte Therapie übernimmt. Das hängt u.a. von den von ihm benötigten Medikamenten ab.
WETTLAUF GEGEN DIE ZEIT
Keiner der Ärzte, die wir getroffen haben, wollte uns eine Angabe über Florian’s Lebenserwartung geben, nicht einmal eine Schätzung. Gemäss Artikeln, die ich auf der Website der American Cancer Society gefunden habe, beträgt die durchschnittliche Lebenserwartung für terminalen Rektumkrebs mit Lebermetastasen ohne Behandlung etwa sechs Monate, was die Zeit zu einem kritischen Faktor macht. Wir wollen nicht, dass Florian sein Leben verliert, nur weil wir uns private Operationen und Therapien nicht leisten können. Je länger er auf öffentliche Behandlung wartet, desto höher ist die Wahrscheinlichkeit, dass sich sein Krebs auf andere Organe ausbreitet, wodurch sich die Chancen auf eine Heilung weiter verringern. Florian kann es sich nicht leisten, weitere Tage ohne ärztliche Behandlung zu verlieren. Seit Dezember letzten Jahres hat er bereits 8 kg abgenommen. Er hat den Appetit verloren, sein ständiger Durchfall und seine rektalen Blutungen sind die grössten Sorgen für uns.
OPTION SCHWEIZ
Wir haben auch die Möglichkeit in Betracht gezogen, in die Schweiz zurückzukehren, deren Staatsangehörigkeit er besitzt, und ihn dort behandeln zu lassen. Wir wissen jedoch nicht genau, ob die öffentliche Hand/Krankenkassen alle seine Chemotherapie- und Zieltherapiebehandlungen abdecken würden. Wir wissen auch nicht, wie schnell er operiert und behandelt werden könnte. Angesichts der Dringlichkeit darf er keine weitere Zeit verlieren, um auf die Behandlungen zu warten, die er benötigt. Ohne Arbeit und ohne Einkommen würden uns in der Schweiz zudem weitere Probleme bevorstehen. Sein jetziger Abreitgeber zahlt ihm 120 Krankheitstage mit vollem Gehalt und zieht dann das Gehalt (2/3 und 1/3) für weitere 120 Tage ab. Danach haben wir kein Einkommen mehr zum Leben. Wir wissen noch nicht, ob er während der Chemotherapie arbeiten kann und wenn nicht, ist es unwahrscheinlich, dass er innerhalb von sechs Monaten zur Arbeit zurückkehren kann. Wir müssen nicht nur alle seine medizinischen Ausgaben decken, sondern benötigen auch Geld, um unsere allgemeinen Ausgaben einschließlich Miete, Essen, Versorgung und Transport zu decken. Die Schweiz ist eine Option, aber eine die mit vielen Unsicherheiten verbunden ist. Umzug bedeutet auch eine große Wette auf sein Leben, seine Lebensversicherung, die von seiner Firma bereitgestellt wird, wird abgeschnitten und das ist es, was er am meisten für uns besorgt. Er ist sehr besorgt, wie wir überleben können, wenn er stirbt, da er der einzige Brotverdiener in der Familie ist.
Wir setzen nun alles daran, die erforderlichen Mittel zu beschaffen, um Florian eine Privatbehandlung in Hong Kong zu ermöglichen und dadurch seine Überlebenschancen zu erhöhen. Deshalb bitten wir Sie aufrichtig, Florian in dieser schwierigen Situation zu unterstützen. Jedes Bisschen zählt. Doctor suspected he has blockage in the colon and they have been doing X-ray daily and. . . . .
