Please help a recently disabled grandmother avoid home foreclosure

I am a 61 year old grandmother of five. I recently became disabled and I fell behind on my mortgage. The mortgage company is now threatening to foreclose on my home of 31 years. If I receive the help to catch up. I am sure I will be able to get back on track. I am not use to asking for help. I am usually the one that gives it. I was a nurse for 28 years at a nursing home before becoming disabled. I am praying that someone would find it in their heart to please help me to save me home.
