Overweight & unhealthy need surgery

hello my names Madison I'm 23 years old my height it 5,2 and I weigh at 112kgs a year in a half ago I weighed at 131kgs but since then I have not been able to get my weight off and it's causing me bad health problems I also have polycystic ovaries syndrome which can also make you put on weight, I found out when I was 16 and just kept stacking it on.. I don't have much of a life because of my weight I don't have a job because my body and my feet just can't stand for long periods of times. My weight have caused me bad depression and I really want to make a future for myself I want to be a mother one day and I can't at the weight I am...

Please help me I really need something to help me want to keep going and this is it I need to be healthy & active I just need a start ..
