Mental Health for Homeless Man

I stumbled across a middle aged man. He is currently homeless because he sufferes from very severe mental illnesses. I would like to get him back on his feet. I would like to do this by: - Making sure that he has clothes. - Giving him food for 1 year. - Giving him lodging for 1 year. - Help him purchase the proper medications that he needs to be taking. - Find him some professional help. - Lastly, he needs perscription glasses.  His situation is dier, and he needs help right away.
