Medical costs

i am moti.I read in class 10.I am 16 years old..I was born with problem,My lips was not good.Then my parents decided to operation my lips.Doctors did the operation very good.My lips became good.But one day,our homemaid took me to feed food.But i was felt.As a result I need to again operation.As a result my lips again damaged.In that time my age was 4/5 year old.Now 12 years have been passed.I visited a doctor,who said that my leaps needs to operation and my tooth needs to be fixed.Doctor said,a piece of skeleton will be attached to my mouth,as a result all the tooth will be settled.My tooth is not in good position thats why my voice is not good.All the persons can't understand what I say.Doctor will also try to fix my lips.Doctor told that it needs more than 400000 bdt(5000 us dollar).Please help me😟😟😟😟😟
