Medical Bill Struggles

We have been struggling for a few years now with medical bills we are just under the 200% poverty level and the hospital refuses to work with us on reducing our amount due or wiping our bills off the record completely(like they are supposed to when you are lower income) all because we have a retirement plan, they want us to pull a loan against our 401k to pay them off and then make payments back to it which still leaves us paying out which is our current struggle. My husband has been having back problems for about 10 years now has had 3 surgeries on one disc and has several others that need surgery too, he is currently fighting off a fusion but is on loads of medications. He works everyday at a full time job as a machinist and supports his family well he is also having to get cortical injections every few months which is running us around $1000 each time after insurance(BCBS which pay alot for and they still dont cover as much as one would hope) at this rate our bills will continue to go up, the monthly payments are getting higher everytime he has another procedure. We have been able to set our own amount to be paid on the bills but now they have this new system where it determines how much you pay per month depending on how much you owe, this has caused our monthly payments to triple and hes scheduled for another set of injections at the end of June which means the monthly bill will go up even more. We pay our bills like we are supposed to we dont ask for help unless we absolutely need it and it has got to that point. If you can help in any way anything will help all we want to do is get out of this bit of debt so he can continue to get the medical help he needs, we are a family of 5, one is in college he is 19 and one is 15, and the youngest is 7, I am a stay at home house wife with a titanium rod from knee to hip from a car accident and struggle with severe pain in my feet from no arches, we do our own repairs on our vehicles and our home and can't cut anymore corners than we already do.
