My name is Jose Alfaro. I am a 46-year-old man from Mendoza, Argentina. I am writing to you today to ask for your help in funding the surgery I need to fix my herniated disks.

Several years ago, I was working as a parking attendant in Manhattan when I was accidentally struck by a car. I was rushed to the New York Hospital division of New York Presbyterian Medical Center, where I underwent a series of tests. The doctors found that I had suffered a severe herniated disk in my lower back.
