Left alone without a carer

Four and a half months ago I discovered that my ex partner was having a seven and a half month affair with an older man at the time she was my carer because of my physical and mental health problems. I suffer from borderline personality disorder along with untreated autism spectrum disorder and ADHD. I also have spinal spondylitis in which I live with chronic pain everyday and there is no cure for as well as permanent nerve damage due to the removal of a rare tumour that has left me unable to get around like I used to which leaves me at times bed bound due to the severity of my spinal pain. I have a new partner in America called Sarah originally British born who was an only child and was raised by her uncle after both of her tragically died and has since been living with him. Sarah is now 33 years old and wishes to join me and be my carer, but now I'm on a low income due to the my state of of my health I can no longer work and am desperate to get her here to the uk. Everyday is just as hard as the last and my physical and mental health is deteriorates due to being as far apart as we are. Until we are finally united, I pose a risk to my own health and well-being. Which is why I have turned to anyone who knows what it's like to be in the position I've struggled with these recent months, and left to fend for myself. Which is why I ask for your kindness and help with helping my partner and carer come home to start a new life but mostly to help me get rehabilitated and ease my struggling with everyday life.
