Brain Surgery Survivor may not survive due to Hospital Promissory note Debt

I suffered what the doctors called a cerebral infarct. My company did not help me with the hospital bills and most of what paid the hospital bill was through charity. I lost my balance and needed assistance with standing up and walking. I am the only one who earns for the family and I am a father of 3 little boys, 2 of which I havent seen for the past 4 years due to some conflict. Now, I am afraid that the stress that may have caused my neurotic stroke incapacitates me again.

I am unable to earn and work for almost 2 mos now since I was discharged. Upon discharge, i signed an agreement or promissory note with the hospital for 104,000php so that they can let me go home on my son's birthday last feb 2 2019.

I was operated on last Jan 9 of the same year. 3 days ago, the Hospital Collections dept called and told me to settle the promissory note this March 30th. I am still without money and job to get the money from. I feel desperate doing this but i dont know where else to turn.
