SPINE SURGERY – MINIMUM INVASIVE PROCEDURE - REHABILITATION
TO WHOM IT MAY CONCERN
This is to certify that I have seen Ms. Agnes Mtete, a 49 years old female patient on
31th August, 2017 at Regency Hospital, Dar-Es-Salaam, Tanzania.
She presented with complaints of Chronic Lower back pain and radicular pain in right lower limb for last 01 year. She also reported having limitation of standing and walking.
She diagnosed having L4-5 Degenerative Disc Disease, Annular Tear. She requires undergoing Interventional Spinal procedure for the same. The proposed procedure entails Disc FX RFA Annuloplasty at L4-5 level to relieve her pain and improve her symptoms. She requires travelling to The Spine Clinic, Ahmedabad, India for the above mention procedure.
She will require staying in hospital for 03 days and 10 days out of hospital for postoperative recovery.
Kindly assist her for the same.
