Help an HIV patient get the proper tests and medication

I am HIV+ and in my country we're very stigmatized and discriminated against, proper testing of CD4 and viral load is very expensive and I have no healthcare, Also the medication available for free is not effective, I am feeling worse and I have no means to confirm if I'm getting better or if I'm having complications, and I have no way of knowing if the medication I take is actually working but my symptoms are worsening and I fear the worst... Please help.
