Dear Sirs / Ma’am,

Greetings!

My name is Sorily G. Zaballas, 37 years old, grew up in Tinambac Camarines Sur, I am wife of Jojo B. Zaballas who grew up in Talisay Daet Camarines Norte, both born April 23 with one daughter @ 4 year old.

I am currently working as Admin Secretary of the Non-Profit Organization. My husband Jojo was a contractual employee of Makati Development Corporation (MDC) with a position of HR Associate but since March 2018 he stopped for work due to his health condition. I am the only working and earner of the family, my take home income is ranging to 15,000 but our expenses for basic needs and medications is more than my take home income.

As part of the history, my husband Jojo since a child has a heart enlargement. Year 2014 he was hospitalized which consider as his first heart attacked and diagnosed with a Heart Failure (secondary Dilated Cardiomyopathy) his heart ejection fraction was 29% the normal is 55-77%. By then he was advice to not engage in a strenuous activities and with a lifetime medication with an average of P5,000-7,000 per month.

Last September 29, 2017 around 8PM, My husband Jojo had a second Heart attacked he was congested and rush to the nearest hospital in Makati Medical Center same diagnosis and was advice to undergo Coronary Angiogram but due to financial incapacity we cannot afford to have the procedure in the above mentioned hospital, we’ve decided to transfer to Philippine Heart Center last October 2, 2017 and was able to discharge and sent home October 05, 2017. As final prognosis my husband Jojo classify with a “severe heart failure” DILATED CARDIOMYOPATHY his heart ejection fraction is 23% compare to a normal of 55-77% as per the results of Coronary Angiogram Jojo's heart status disorder already and only medicine can sustain him. While at home Jojo always in pain due to some complications and at night and frequently stop on his breathing by then he was advice to visit a pulmonary Doctor to check if he has sleep apnea. October 26, 2017 that supposedly only check up to a pulmonary Doctor my husband Jojo had chest pain and irregular heartbeat and advice for close monitoring he was in the ICU for 7days and he is a candidate for primary prevention of sudden cardiac death by Implantable Cardioverter-Defibrilator (ICD) implantation with estimated cost of P700,000 to 1M expenses. As days goes by Jojo's heart pump is getting low (ejection fraction at 20%). Because of our financial struggles it took us five (5) months preparation to gather and generate funds for the procedure by the help of some institution and good Samaritans but still lacking because until now we owe bills to the hospital and our left and right debts and the monthly medication average of P35,000. Fortunately, the procedure for ICD Implantation successfully done last March 22, 2018.

July 01-July 5, 2018 another battle has fought due to some complications my husband Jojo was hospitalized again due to difficulty of breathing, severe gout, Low BP, Low Electrolytes, Low Blood sugar, and Low Calcium Magnesium.

February 25, 2019 another battle has fought, 4th time to be in the Cardio Vascular-ICU of Perpetual Help DALTA Medical Center. It was like again a rollercoaster ride for us last February 24, 2019 because He collapsed around 3PM while walking outside home, nobody saw him he didn't know what happened, only GOD alone. The LORD is Merciful, He alone can do Miracles. He gave Jojo another chance to Live Life to the fullest. After 5days in the hospital his BP was normal, his CT scan results was clear, his oxygen was normal except the Weak Heart .

"Life can change in a minute but through Prayers, our GOD can do miracles."

December 16,2019 Jojo had his quarterly check up everything was fine except for low sodium, the Doctor required him to undergo 2Decho to check if there is changes in his ejection fraction so I scheduled it December 27, 2019.

December 17, 2019 - record shows that there is an "arrhythmia" in his heart "very fast heart beat". 200-260/bpm but the ICD function only on cardiac pacing no fire or shock happened. Jojo's condition at that time was very normal, he drive, we attended Christmas gatherings.

December 19, 2019 - while unloading stuffs of a friend at the lobby of hotel in MOA around 1pm he collapsed for 10seconds we thought he was hungry, we had lunch then he got back to normal. Everything turned to normal at 7pm we had simple dinner for our daughter’s birthday as if nothing happened.

December 20, 2019 - he woke up 8am after praying together, he prepared breakfast, I'm preparing for work at that time. Supposedly he will get water from the refrigerator but his ICD suddenly fired and He collapsed and pass out for a few seconds, I was on his back at that time and rescued him.

My husband Jojo turned to normal but very weak he told me, he has low heart beat and he need oxygen. I prepared the standby oxygen, check his bp and oxysat very normal, i called the Doctor and ask what to do. I also asked help to our neighbors and friends. Jojo was then conscious so I decided to request an ambulance to my boss coordinator. "The LORD provides people to be with us in difficult times."🙏

Upon arrival of the ambulance the Doctor/nurse checked Jojo's blood sugar it shoot up to very high level, injected insulin to normalize. Until we reach the Emergency of Perpertual Help Hospital, mu husband Jojo was so weak but he is a fighter "no complain" even if he is struggling and in pain or frustrated with life. He always remind me to take care of our daughter Hannah in time like this. The “ICD implantable Cardioverter Defibrallator” was a great invention, guaranteed can back up and be the instrument to save lives of thousands who has severe heart failure "CARDIOMYOPATHY".

Every unfortunate event of our lives is like a roller coaster ride, it's a test of FAITH and PATIENCE.
Being a wife, a mother to a 4yr old daughter, a working Mom it calls high sacrifice. I need to remind myself to be strong despite of my down emotions, I'll do everything for my husband and my daughter.

We spent 5 days in the hospital, we arrived home 8:30pm December 24, 2019. Lord Jesus gifts to us is not to spend Christmas in the hospital. He always listen to our prayers.

My husband Jojo is now positive to a Type 2 Diabetes aside from his severe Heart failure Dilated Cardiomyopathy. There are additional lifetime medications combination of oral and insulin everyday.

My husband condition will need prolonged long term treatment and medication. Meantime he is coping and gaining strength at home and looking forward to GOD’s Miracles for a Healthier Heart everyday.

We’ve been struggling with financial difficulties since he was diagnosed with Heart Failure and we cannot sustain all the expenses for his medication as well as our basic needs even if, we got some donations from a generous bosses, relatives and friends.

In line with this, I am humbly requesting for a financial help and support to provide the needed medical and financial necessities for my husband “Jojo”. Your generosity would be a great help to us.
