family’s home is falling apart and wife has a rare terminal illness, please help

Last year my wife was diagnosed with PAH disease, it's a progressive disease that has no cure, I have 3 children and my wife and I living in a older mobile home and as soon as my wife got sick our home started falling apart, my job cut my wages plus hours and I can barely afford to pay the bills let alone do any maintaince on the house, our electric has totally quit on one side of the trailer and the water pipes under the house rupture on a weekly basis. I'm also behind in utility bills, house payment, plus property taxes. I'm afraid if I don't get help soon we will be homeless, anything would be greatly appreciated.
