Hi my name is Carlo Jeffrey Liwanag, I live in Davao City, Philippines, and we are currently on a citywide lock-down due to the COVID-19 virus pandemic. I am setting up this medical fund raising campaign for my 3-year old niece, Khannah, who will undergo a major medical surgery on May 4, 2020 at 8:00 AM GMT+8.

Below are the details of Khannah's journey as relayed by her mother, Guen.

It started in the morning of April 24, 2020, when Khannah has been complaining of a lower abdominal pain. We gave her breakfast thinking she was just hungry but afterwards, she started throwing up all that she ate. After doing our home interventions, the pain was not relieved so we decided to have her checked in the nearest hospital. Blood test and urine sample was ordered. Her white blood count result came back high. She was given an injection for the pain. The doctor advised that if the pain persists, we need to send her to a bigger hospital for further treatment.

While at home, her abdominal pain was temporarily relieved for 2 hours. During lunch, she had small amount of rice and then she threw up again. We let her stomach rest and gave her fluids from time to time. Evening came, and pain is still recurrent on the lower right part of the abdomen. Right there, we decided to send her to a bigger hospital which is 30 minutes away from our house.

Her Dad accompanied her to the hospital while I stayed home with our baby boy. In the ER, they took blood tests, performed xray and admitted her on IV fluids for observation. She was not allowed to eat anything and no pain medication for the next 24 hours to rule out Appendicitis. She was in pain all night and we were helpless seeing her suffer until the doctor sees her in the morning.

The next day, she was still complaining of severe abdominal pain and she want her Daddy to carry her all the time. She was moaning and crying and screaming over the phone or whenever I call to check on her. I can't help but cry seeing her in so much pain.

Finally, the pediatrician came and ordered for ultrasound. The result came out and she explained that the pain could be due to a block in the bile duct. A much hi-tech diagnostic test was suggested. Afterwards, they gave her IV pain medication and she was relieved for a while but the pain recurred after 10 to 15 minutes. The doctor again ordered for another medicine and still she was relieved for a few minutes only. They gave her three shots of pain reliever and her Dad was carrying her all the time until she falls asleep and that's the time he can put her in bed.

The Pediatrician referred Khannah to the surgery doctor and came to see her the next day. Her abdomen was already bloating from 42 cm to 48 cm. She was vomiting saliva and gas from so much pain. The surgeon ordered for CT scan and was done on the same day. A tube was inserted in her nose to relieve gas and a stronger pain medication was given every 6 hours as needed. The nasogastric tube insertion was the hardest, her Dad couldn't bear looking at her in such agony. After insertion of NGT the pain finally subsided and she was able to sleep through the night for the first time in 2 days.

The next day, April 27, 2020, while I was on the phone, the surgery doctor explained to us the CT scan result revealed a congenital anomaly in the biliary tract of our daughter. He suggested surgery but since their facility is not well-equipped to cater this kind of surgery, he referred us to Pedia Gastroenterologist who is only available in Davao city which is about 4 hours away from where she was admitted.

The hospital started the referral process with communication and coordination to Davao City hospital. It was intricate and rigid because we were in the midst of pandemic. We needed ambulance which is also very hard to find at this time, plus an accompanying nurse to transfer my daughter to Davao City with a clearance from the infectious disease specialist for Covid negative test screening. We have called a lot of people and with the help of our family and relatives, we were able to successfully do all this in a day. And we could only thank them and be grateful for their presence during this difficult time of our lives.

On April 27, 2020, Monday, at around 9am, my daughter and husband were mobilized to Davao City via ambulance. I was able to see her when they dropped by the highway to get some things and she was surprised to see me there. She was asking, "Mommy, where are you going?" then I said, I am just gonna give Daddy your things and kiss you baby! " I wanted to cry but I need her to see that I am brave and strong like her.

They traveled to Davao and were transferred directly and safely to DMSF (Davao Medical School Foundation) Hospital Inc., Admission process was done. They did another IV insertion for my her and another set of blood tests.

The Pediatric Gastroenterology Specialist saw her that day and assessed my daughter. She is already without pain. They reviewed and studied her case. CT SCAN was re-evaluated and correlated with the impression of the previous hospital where she was admitted.

They referred her to a Pediatric Surgeon and he explained to us that our daughter has a congenital anomaly in her biliary tree which is rare and happens to 1 in 150,000 to 200,000 children. He said that surgery needs to be done as soon as possible to avoid further trauma and complications in her bile duct system and so that her liver won't be affected. They need to cut her open under a general anesthesia and the operation might last for four long hours plus recovery. The surgeon told us that she needs to stay another week after the operation for observation since she will not be allowed to eat for 4 days and she needs to be monitored closely post operatively.

Not even in my wildest imagination did I ever think of my most fragile, jolly and happy three-year old daughter to undergo a major operation at a very young age. I also could not risk either letting her go out of the hospital without any treatment amidst the Covid crisis that we are experiencing nowadays. I could not help but cry in prayer everyday since the day she was admitted up to this very hour while carrying my second born 5-month-old infant thinking I will not be there to hold my daughter's hand during these precious times that she needed me. I am just relieved that my husband is there to take care of her. Looking at the brighter side, their bond will grow stronger than ever.

My husband, who was an ex OFW, just resigned from his work last March because their company could not afford to compensate them for five months already, and me being a Housewife who just gave birth 6 months ago, our funds have been squeezed and drained when we relocated to our new house up to this predicament and operation of our daughter tomorrow 8 in the morning of May 4, 2020.

These past week has been very challenging for us, and even if she is still in the hospital, she still managed to see the good. She appreciate simple joys, laughter, played with her baby brother on the phone and even sang her fave songs. She may be little but she is fierce, she may be fragile but she is strong, she may be wounded but she is courageous.

And there is nothing I want more than to see her growing up healthy and without pain. All I pray is to see her home with me, making funny faces and singing her favorite songs. Please help us pray that her surgery and recovery will be successful and smooth so she could come home to us real soon. Your prayers will go a long way for our brave little girl.

Day 4 Post Surgery

Our brave Khannah is recovering well. 😘 She was on liquid diet yesterday well tolerated and is shifting today to soft food. She is still on IV antibiotics and JP drains with 10 - 15 cc light pinkish fluid. Urine output is optimal and she can now sit and stand. . . . .

Clinical Update:May 6, 2020NGT tube and Urinary catheter was removed today. Still with a JP drain. We are waiting for the biopsy result and hopefully she can start eating in a few days.Thank you, thank you, thank you so much to all who donated and prayed and are still praying

Day 0 Post Surgery Update

May 4, 2020 Update: After almost 7 hours inside the operating/recovery room, our Khannah is finally out of the OR and is transferred to the PICU (Pediatric Intensive Care Unit). As per the surgeon, the surgery went well. They did not find any cyst inside the liver but they still. . . . .
