I am starting this account to ask help for my husband Chris' medical treatments which include chemotherapy and laboratory tests.

Chris is a funny, responsible, caring, and loving friend, husband, and father of our two children - Calix,6 and Yomi, 4. If you are close to him, you'd know how he his always willing to help out anyone with the best of his abilities. But this time, it is he who needs a little of your help.

Last May 2016, he felt an unusual shortness of breath and his X-ray and CT-Scan showed that he has pleural effusion or fluid in his left lung and an unidentified mass in his chest. He was confined to have the fluid drained and the mass subjected to biopsy. A few days later, he was diagnosed with Stage 3a Non-Hodgkins Lymphoma.

Although the doctors told us that his type of lymphoma is cureable, he would still need to undergo 8 cycles of chemotherapy and a few rounds of radiation. He would also be given Rituximab a drug made specifically to target CD20 B-cells. However each dose of Rituximab costs Php 90,000 and he will be needing one per cycle.

His salary and mine combined is not enough to cover the expenses. Unfortunately, he was already advised by the doctors to take a leave from work so as not to put his health at more risk because his immune system is low.

Because of the amount of his medical bill, we're hoping a little financial assistance will also help reduce Chris' stress level so he can concentrate on getting better.

This may be a long shot but any amount will go a long way. We will greatly appreciate any amount the you will generously share.

Through all these hardships and struggles, Chris has kept a positive attitude and continues to do all that he can to care for his kids and family.

A guarantee letter is a certification issued to hospitals for a particular charity patient under the PCSO medical assistance program where the agency assumes the obligation of settling the cost of hospitalization, including the medicines, medical, surgical or blood supplies, and diagnostic procedures. OBJECTIVES General : Restoration of social functioning (Physical recovery) through medical assistance Specific : Provide assistance for hospitalization expenses, laboratory procedures and purchase of medicines, chemo drugs, dialysis solutions, antibiotics, implants, devices and other medical needs. DOCUMENTATION REQUIREMENTS Letter Request to Chairman/General Manager Medical Abstract Bill/Quotation/Costing from Hospital Pharmacy/Supplies Laboratory Request/Medicine Prescription Endorsement/Acceptance letter from Hospital Social Services/Credit Collection Office The families may write to Atty. Jose Ferdinand Rojas for medical assistance and bring the following: The letter request shall be addressed to: Atty. Jose Ferdinand Rojas PCSO General Manager C/o Aida Once the required documents were accomplished, the patient or their immediate relatives can bring them to the: 4th Floor, Sun Plaza Bldg. Shaw Blvd., Mandaluyong City PCSO will always be there to help but the assistance is socialized depending on the financial capacity of the patient's family. Hopefully, it will help alleviate the burden caused by the medical condition. Letter of request Medical Abstract with signature and license number of the attending physician Billing Statement and treatment protocol Once completed, bring to her office at the 4th floor, Sun Plaza Bldg., Shaw Blvd., Mandaluyong City. Just tell them that Lawrence See, friend of Kirk Arambulo, both of Junior Chamber International (JCI), referred you.
