Can’t work temporarily due to back injury

I am a caregiver for a company here in WV. I injured my back caring for one of my clients. My boss waited almost 4 week to report it to the WC insurance agency. Then it took 2-3 weeks to get it started. I was told that my employer has been contacted by the WC insurance company to send proof of income to them. I got the 4 months worth of check stubs and turned them in to move the process quicker. I was informed today they need them for the whole past year now. They requested this info from him again and since he has not been helpful up to this point I don't think he will be now. I have put out every single penny and paid all my bill through March 2017, but now I am only receiving 100.00 a week for payment that won't even pay my rent. I am going to be homeless and loose everything very very soon. My baby boys Rocky, the boxer, and Elliott the cat and I have no place to go. Please please help us.
