JUDALYN VILLAHERMOSA CARBONERA, 64 years old was diagnosed with CHRONIC KIDNEY DISEASE STAGE 5 SECONDARY TO DIABETIC NEPHROPATHY. She started hemodialysis last April 2015 up to present. Her scheduled hemodialysis is thrice (3x) a week. Aside from HD she also have an oral medications for her diabetes & cardiovascular disease.
