Young Timorese woman immunodeficient with no previous treatment. Temperature 40 Celsius. Mouth and throat filled with white sticky lesions. Chest x-ray has bilateral perihilar infiltrates. Respiratory rate is 60 per minute. Monumental challenge anywhere but especially where many tests and treatments are unavailable. Fortunately after 4 days she seems to be improving.

Diagnosis: HIV/AIDS, Pneumocystis pneumonia, TBC, Candidiasis, Sepsis.
