Admitting Diagnosis: Acute complicated UTI (cystitis) Pre B cell ALL, early relapse with CNS involvement s/p 6 cycle of hyper CVAD, s/p intrathecal chemotherapy (Day 11 post chemotheraphy), Perianal abscess s/p resolved, DM type 2; Febrile neutropenia; r/o Clostridium difficile associated diarrhea; r/o adrenal insufficiency; AKI probably secondary to 1. Sepsis; 2. Dehydration; hypovolemic hyponatremia

Final Diagnosis: Pre B cell ALL, early relapse with CNS involvement s/p 6 cycle of hyper CVAD, s/p intrathecal chemotherapy; AKI stage 3 secondary to sepsis and dehydration; multiple electrolyte imbalance secondary to poor oral intake; steroid induced hyperglycemia; Septic shock secondary to e.coli bacteria, secondary to complicated cystitis-resolved; Neutropenic typhilitis-resolved; Cellulitis right posterior axilla; Oral herpes simplex infection-resolved; Febrile Neutropenia-resolved

-Chemotherapy is requested by the Hematologist to continue and scheduled after 2 weeks but depends as well within his condition
